I want to develop the ruler to measure the length on dicom image. But I do not know the scale between real world and dicom image.
Sometimes the pixel spacing of dicom file is zero.
Does somebody know which spacing is the best scale?

The ruler measurement is to measure distance in a straight line between two points within the plane of the image (using Pixel Spacing (0028,0030), Imager Pixel Spacing 1030 (0018,1164), Nominal Scanned Pixel Spacing (0018,2010) or the Ultrasound Region Calibration Module as appropriate for the SOP Class of the image).
Distance should be reported in pixels rather than physical units if there is no spacing or calibration information in the image.
We also provide tools for user to calibrate the ruler such as CalibrateRuler().
